Here's what I submitted: dimensioned site plan showing the 4-ft accessible path, stamped engineering drawings for the awning and side walls, certificate of insurance with $1M general liability naming the city as additional insured, a maintenance and snow-clearance plan, and a lease addendum signed by my landlord. Photos of the current setup from four angles. DPW flagged my first submission because the pinch-point between the parklet and the curb was 42 inches at one spot. I had to relocate one planter. Total turnaround was about 5 weeks once the package was complete.
